Medical Records Requests

Complete the below medical record request form, pay the processing fee, and email the completed form to info@panditmd.com 

Records will only be available on a thumb drive. A pickup address will be provided after processing is complete.  

Please note, records cannot be faxed due to file size and records cannot be provided through other methods due to HIPPA compliance.

Please allow up to 5-10 business days to process the records request. 

Please note, there is a nominal $50 fee for processing records requests.
